Dupuytren’s Contracture Fasciectomy

Dupuytren’s contracture affects men more than women, with most cases in men over 50 and women over 60.

Following consultant assessment and advice, Dupuytren’s contracture fasciectomy is often advised when experiencing any or all of the following symptoms or if other treatments such as enzyme injections have proven unsuccessful:

• Not being able to lay your hand flat on a table, palm down (called the tabletop test)
• One or more small, tender lumps (nodules) in the palm
• Thick bands of tissue under the skin in the palm of the hand
• Pits or grooves in the skin compressed by the contracted finger/s.
• Fingers are pulled forward
• Your hand is not able to work as well

The surgical approach depends on the position and severity of the Dupuytren’s contracture, the condition of the skin and if there has been any previous surgery. Surgical options may include:

• Open or needle fasciotomy – an outpatient procedure performed under local anaesthetic. A sharp blade or a very fine needle is inserted into the fibrous bands of the palm or fingers to divide the thickened connective tissue and release the tightness in the hand that is causing the contracture.
• Dupuytren’s fasciectomy –performed under general anaesthetic as an day case procedure. An incision is made in your hand and the affected connective tissue is removed. A skin graft may be required to seal your wound. Fasciectomy is used to treat more severe cases.

The operation may take 30 minutes to an hour (or more), depending on the surgical method carried out and the number of fingers involved.
